A community health assessment (CHA) is a systematic examination of the health status indicators for a given population that is used to identify key problems and assets in a community.
The ultimate goal of a community health assessment is to develop strategies to address the community’s health needs and identified issues.
Non-profit hospitals are required to complete a CHA and develop a Community Health Implementation Plan (CHIP) to address identified needs every 3 years.
The North Suffolk CHA kicks off in October 2024 and went through several rounds of community engagement and feedback before completion in October 2025. Stay tuned to this page for progress updates and to learn of new opportunities to be involved.
