The North Suffolk Public Health Collaborative (NSPHC) is conducting a Community Health Needs Assessment to explore the community health strengths and challenges that matter most to people in our communities – Chelsea, Revere, and Winthrop.
What is a Community Needs Assessment?
A community health needs assessment (CHNA) is a systematic examination of the health status indicators for a given population that is used to identify key problems and assets in a community. The ultimate goal of a community health assessment is to develop strategies to address the community’s health needs and identified[LB1] issues.
Non-profit hospitals are required to complete a CHNA and develop a Community Health Implementation Plan (CHIP) to address identified needs every 3 years. See the 2019 North Suffolk CHNA Report here!

The North Suffolk Public Health Collaborative (NSPHC) is conducting a Community Health Assessment to explore the community health strengths and challenges that matter most to people in our communities – Chelsea, Revere, and Winthrop. The purpose of this survey is to hear directly from community members like you. The results of this survey will be analyzed and shared back with the community, and will help NSPHC, our partner organizations, and community members to take action to positively change the factors that influence people’s health.
